Frequently Asked Questions on DME Permit Renewal

 

Q:  When does my permit expire?

A:  Permits expire December 31st of each calendar year and must be renewed annually.  There is a 60 day grace period to renew after December 31st.  The grace period ends March 1st and the online renewal is no longer available.

Q:  What if I have missed the deadline to renew?

A:  If the permit is not renewed by March 1st, late penalties will apply to renew.  Access to the online renewal will no longer be available, so you must complete the DME application (click here to access the DME application.)

Make sure to select LATE RENEWAL at the top and then mail it in to the Board’s office along with the late renewal fee. All late renewal applications must be received or postmarked no later than March 31st. After April 1st, the permit will be CLOSED and permit holders are required to cease and desist all business or dispensing until you have a valid permit.

Q:  How do I update the DME address?  Am I required to update before renewing?

A:  Yes, address changes are required prior to renewal.  Please complete and submit the DME ADDRESS CHANGE FORM (click here).  Please allow 3-5 days to process.

Q:  What updates can permit holders make on the online renewal form?

A:  Permit holders can update the email address and the business/fax numbers.

Q:  How is a Person-In-Charge (PIC) change processed during the renewal period?

A:  All PIC changes are required to be processed before the permit can be renewed.  Once the change has been updated, the PIC is allowed to renew the permit.  Please allow 7-10 days to process a PIC change. (CLICK HERE FOR PIC CHANGE INSTRUCTIONS).

Q:  Can I print a duplicate receipt?

Q:  How do I update an Officer or Director on the DME permit?

A:  Officer or director changes, with no controlling interest in the business, should be emailed to Wendy Watson.  Make sure to include the permit number, name of business, and the information concerning the update (i.e. who needs to be replaced).  Here’s an example; Tom Jones, President, will be replaced by John Smith effective 00/00/0000).

Please submit a corporate chart if it is available.

Q:  The DME permit has had an ownership percentage change.  How do we notify the Board?

A: Please reference this link for the rule concerning an ownership change: http://www.ncbop.org/LawsRules/rules.1600.pdf (scroll down to .1603 - "WHEN NEW PERMIT REQUIRED").

If the rule does not apply (less than a 50% change), email the percentage change information to Wendy Watson.  Be sure to include the permit number, the name of the DME, and the new percentages.

If the rule does apply, then a new DME application is required (click here to access the DME application).  Make sure to complete the entire application including items #6 (ownership table) and #7, and select TRANSFER OF OWNERSHIP at the top of the application.  Please mail in the completed application and fee at least 2-3 week prior to the transfer.  The Board does not post-date or pre-date transfers of ownership.

Q:  How do I close the facility if I do not plan to renew the permit?

A:  Submit written notice to the Board’s office, with attention to Wendy Watson.  Include the permit number, name of business, effective date of closing, and the name and location of where the patient files/records are being and/or have been transferred to. Make sure to return permits (certificates) as referred to in Rule 21 NCAC 46 .1608 (c) (scroll down to .1608 - "DEVICE AND MEDICAL EQUIPMENT PERMITS".)
