The North Carolina Board of Pharmacy received information that a John Richard Clark, a/k/a John Richard Joseph Clark, applied for a job at a North Carolina hospital pharmacy using the name and credentials of a currently licensed North Carolina pharmacist.

Mr. Clark provided the hospital with a resume, a copy of the pharmacist’s license 2008 renewal form, and university transcript displaying all of the pharmacist’s educational information. Mr. Clark provided a Shelby, North Carolina address and phone number.

Law enforcement is investigating this case.

If Mr. Clark, or someone matching the photograph but using another name, has contacted your facility or applied for a pharmacist position, please contact the North Carolina Board of Pharmacy Investigations Office at 919-246-1050.

John Richard Clark, a/k/a John Richard Joseph Clark, is not – and has never been – licensed as a pharmacist in North Carolina or, to our knowledge, in any other state. The Board has information indicating that Mr. Clark was licensed as a pharmacist intern in Georgia from 1995 until 2000. The Board has also received information indicating Mr. Clark has a criminal history of controlled substance violations and previously served prison time in Georgia.
